Question brief / Your conversation
Take your questions into the room.
A question written down is easier to keep hold of. Use the ones that fit your concern, and add your own.
Bring the problem you want help with
Start with what has been bothering you and what you hope to change. You can come to the visit without having chosen a drug.
Before the visit: put it into words
- What is hardest for me at the moment?
- What have I already tried, and what happened?
- Which medicines and supplements am I taking?
- Which health changes or past reactions do I want to discuss?
- What would a useful outcome look like in daily life?
Bring your own list. If a word is unclear, ask the person assessing your concern to write it down and explain it.
Make room for a real conversation
Some online questionnaires give everyone a set of questions and answers, with no way for the clinician to ask more. The Federation of State Medical Boards says that questionnaire alone is not enough to decide treatment, in guidance checked September 26, 2026.
This advice is for medical boards; it is not one law for every state. A questionnaire can be different if a clinician can ask follow-up questions and get the details needed for your care.
Ask: “Can I speak with the person deciding my care and ask my own follow-up questions?”
During the visit: ask about the decision
Compounded describes a medicine prepared to suit one patient’s particular need.
FDA, short for Food and Drug Administration, is the U.S. agency responsible for reviewing medicines.
A compounded medicine lacks FDA approval, as checked September 26, 2026.
- What might be causing my problem?
- What options fit, including options without a medicine?
- What is the exact medicine? Is it a tablet, an injection or something else?
- What evidence fits people with my concern?
- If it is compounded, why does a ready-made medicine not meet my need?
- What examination or tests would help decide?
- Which pharmacy will prepare or supply it?
Make sure you can explain the plan before leaving
Explain the plan back in your own words, and ask about anything missing before the visit ends.
Before agreeing: ask about what comes next
- What changes or problems mean I need to contact you?
- How can I get questions answered between visits?
- When will we talk again and review the plan?
- What is included in the bill, and what is separate?
- How do I end any ongoing payments?

This page keeps the notes on that device and does not send them anywhere. Anyone using the same app on that device may be able to read them. Leave out private health details.
If you delete this website’s saved data in the app’s settings, your notes can disappear. Use “Erase saved notes” below when you want to remove them.
If the controls are dimmed, keep the list on paper instead.
